*Death Records-Active Duty or Retired Veteran - First copy is FREE, each subsequent copy is $6.
The Peru City Clerk's office is the Local Registrar of Vital Records for events that occur in District #50.1 (Western LaSalle County). District #50.1 consists of Deer Park, Dimmick, Eden, LaSalle, Peru, Utica, Vermillion and Waltham Townships, and includes the incorporated areas of Cedar Point, LaSalle, North Utica, Oglesby, Peru and Tonica.

The Clerk’s Office will issue certified copies of Vital Records to authorized individuals only. To do otherwise is a violation of Illinois Law.  Vital Records are not considered public information, nor are they subject to the Freedom of Information Act. No information regarding Vital Records will be given over the telephone under Illinois State Law (410 ILCS 535-Vital Records Act).
Please note, we are only able to produce birth/death records that occured after August 27, 2024. Prior records must be obtained by contacting the LaSalle County Clerk's Office at 815-434-8202.

*TO OBTAIN A CERTIFIED BIRTH CERTIFICATE, YOU MUST BE ONE OF THE FOLLOWING:
(Only birth records that occured after August 26, 2024 can be obtained)

  • The person named on the record if 18 years or older
  • The parent, if their name appears on the Birth Certificate as the mother or father
  • An Agent having a Notarized Letter authorizing access
  • A court order
  • Other Legal Representative, i.e., Attorney acting on behalf of a person, a Power of Attorney, an Agent approved by the State Registrar as a Legal Representative of the child.  Written evidence of guardianship or legal representation is required.

 TO OBTAIN A CERTIFIED DEATH CERTIFICATE, YOU MUST BE ONE OF THE FOLLOWING:
(Only death records that occured after August 26, 2024 can be obtained)

  • The informant listed on the death certificate
  • The next of kin
  • Someone who has a personal or property right interest in the certificate
  • Executor or Administrator of the decedent’s estate

Death records are not public records.  If you are not a relative of the deceased person, a letter or document from the office or agency that needs the death certificate must accompany the request. 


Requests must be made in person at Peru City Clerk’s Office – Monday through Friday, 7:00 a.m. to 3:30 p.m. (except for City holidays).  An application will be provided and must be completely filled out. Payment can be made by cash or check payable to City of Peru, or by credit card (Visa or Mastercard and subject to 3% transaction fee). Acceptable forms of identification are any of the following:

Valid Driver’s License
Valid State Issued ID
Valid US Passport
Valid Military ID
